FinJeHeW on Suomen Telelääketieteen ja eHealth-seuran ja Sosiaali- ja terveydenhuollon tietojenkäsittely-yhdistyksen yhteisesti vuodesta 2009 alkaen julkaisema tieteellinen lehti. Lehti kuuluu julkaisufoorumi-luokkaan 1. Lehden ISSN-tunnus on 1798-0798.

FinJeHeW ottaa vastaan tieteellisiä artikkeleita sekä muuta aineistoa sosiaali- ja terveydenhuollon informaatioteknologiasta, telelääketieteestä ja eHealth-alalta. Kirjoitusohjeet ovat lehden sivustolla. Lehti on pääasiassa suomenkielinen, mutta siinä on artikkeleita, niiden tiivistelmiä ja muuta aineistoa englanniksi. Lehti noudattaa käsikirjoitusten arvioinnissa kahden refereen menettelyä. Lehti ilmestyy ainakin neljä numeroa vuodessa verkkojulkaisuna pdf-muodossa. Lehden kukin numero on vapaasti saatavissa verkosta www.finjehew.fi.

Tiivistelmä

Tämän tutkimuksen tarkoituksena oli arvioida, miten kansallisesti määriteltyä hoitotyön yhteenvedon tietorakennetta ja siirtyvän potilaan hoitotietojen kirjaamisohjeen sisältöä noudatettiin hoitotyön yhteenvedoissa. Tutkimuksessa kuvattiin hoitohenkilökunnan ja lääkäreiden laatimien yhteenvetojen rakenteissa käyttämiä päällekkäisiä tietorakenteita. Lisäksi kuvattiin mitä muita tietorakenteita hoitotyön yhteenvedoissa käytettiin. Rekisteritutkimuksen aineisto (N=87) kerättiin sähköisestä potilaskertomusjärjestelmästä sähköisessä muodossa tutkimusorganisaation yhdeksältä vuodeosastolta. Aineisto analysoitiin laskemalla frekvenssit ja prosentit tietorakenteiden käytöstä. Tutkimusaineiston eri otsikoiden välinen yhteys testattiin khii-toiseen testillä ja odds ration (vedonlyöntisuhteet) laskettiin kuvaamaan otsikoiden assosiaation voimakkuutta. Tämän tutkimuksen tulosten mukaan hoitotyön yhteenvedon kansallisesti määriteltyjä tietorakenteita käytetään hyvin, mutta niiden käyttö ei ole järjestelmällistä samassa yhteenvedossa. Alueellisesti määriteltyjä hoitotyön yhteenvedon sisältöotsikoita käytetään vain osittain. Hoitotyön ja lääketieteen yhteenvedoissa käytetään päällekkäisiä tietorakenteita. Hoitotyön yhteenvedoissa käytetään myös kansallisesti määriteltyyn rakenteeseen kuulumattomia tietorakenteita. Terveydenhuollon sähköisiä tietojärjestelmiä tulee kehittää edelleen ohjaamaan henkilöstöä tuottamaan yhteenvetoja määriteltyjen tietorakenteiden mukaisesti. Terveydenhuollon ammattilaisia tulee edelleen kouluttaa käyttämään määriteltyjä tietorakenteita. Myös potilaat tarvitsevat yhdenmukaisten tietorakenteiden mukaisia yhteenvetoja omahoitonsa tueksi. Tietojen on oltava yhdenmukaisia tiedon käytettävyyden turvaamiseksi ja epäselvyyksien välttämiseksi.

Abstract (English)

The aim of this study was to evaluate how the data structure of a national nursing discharge summary and the contents of nursing records of a transferring patient were followed within nursing discharge summaries. In this study, we described the overlapping data structures of nursing discharge summaries that had been compiled by both nurses and doctors. In addition, we described what other data structures were used. The data (N=87) were collected from the electronic patient record system in electronic format from nine inpatient wards of the research organization. The data were analyzed by calculating the frequencies and percentages of the use of data structures. The link between the different headings of the data was tested with the chi-square test and the odds rations were calculated to describe the strength of association between headings. According to the results, although the national data structure of the nursing discharge summary was used well, its use was not systematic within the same summary, and regional content titles were only partially used. Overlapping data structures were used in both nursing discharge summaries and medical summaries. Non-structured designs, which do not belong to the national structure, were also used. Therefore, health care electronic data structures must be further developed to ensure uniform completion per the determined data structure. In addition, health care professionals must be thoroughly trained to use the determined data structures; patients require uniform summaries that contain clear and usable information to support their own treatment.
