FinJeHeW is a scientific journal published jointly by the Finnish Society for Telemedicine and eHealth and the Finnish Association for Social and Health Care Informatics since 2009. The journal belongs to publication platform category 1. The ISSN of the journal is 1798-0798.
FinJeHeW welcomes scientific articles and other material related to information technology in social and healthcare services, telemedicine, and eHealth. Author guidelines are available on the journal’s website. The journal is published mainly in Finnish, but it also includes articles, abstracts, and other material in English. Manuscripts are evaluated using a double-referee peer review process. The journal publishes at least four issues per year as an online publication in PDF format. Each issue is freely available online. www.finjehew.fi.
Tekonivelinfektioita aiheuttavien riskitekijöiden selittäminen tai ennustaminen potilaskertomukseen tallennetun tiedon avulla en

Abstract
Sähköisiin potilastietojärjestelmiin tallennetun tiedon määrä on valtava. Tietojärjestelmistä saadun strukturoimattoman tiedon hyödyntäminen on mahdollista tiedonlouhintamenetelmän avulla, missä suuriakin tietomassoja voidaan jäsennellä eri keinoin ja saada aikaan jäsenneltyä tietoa tutkittavasta ilmiöstä.Tekonivelinfektio on potilasturvallisuusriski, joka aiheuttaa haittaa ja kärsimystä potilaalle. Pidentyneet hoitoajat ja viivästynyt toipuminen ovat turhia rasitteita myös yhteiskunnallisesti. Tekonivelinfektion ennaltaehkäiseminen kaikin keinoin on ennen kaikkea potilaan etu. Tekonivelinfektioiden seurantarekisterit vapaaehtoisista aina lain velvoittamiin tuovat ongelmia infektioiden määrän ja rekistereiden tiedon laadun arvioinnissa.Tutkimuksen tarkoituksena oli selvittää, voidaanko potilastietojärjestelmään kirjatun ja tallennetun tiedon avulla selittää tai ennustaa mahdollisia tekonivelinfektioita aiheuttavia riskitekijöitä. Tutkimuksen kohteena oli tekonivelinfektion saaneet potilaat ja verrokkiryhmänä tekonivelleikkauksessa olleet potilaat, jotka eivät saaneet infektiota eräässä yliopistollisessa sairaalassa vuosina 2011–2013. Tulosten mukaan erityisesti infektion saaneilla potilailla esiintyi leikkaushaavan vuotoa vielä kotiutusvaiheessa. Lisäksi leikkauksen aikaiseen ja -jälkeiseen potilaan lämpötaloudesta huolehtimiseen tulisi kiinnittää erityistä huomiota. Tämän tutkimustuloksen perusteella tekonivelinfektioita voidaan ehkäistä ja ennakoida huomioimalla riskitekijöitä, mutta lisätutkimuksia tarvitaan. Vaihtelevat kirjaamiskäytännöt vaativat edelleen kirjaamisen kehittämistä ja yhdenmukaistamista.The amount of data stored in patient record systems is vast. It is possible to utilise the unstructured data of the information systems though the data mining method, which allows analysing large masses of data through different methods and enables obtaining structured information about a desired field or topic.Artificial joint infection is a patient-safety risk and will always cause harm to the patient. Longer treatment periods, patient's suffering and delayed recovery also cause an unnecessary burden to the society. Arthroplasty infections prevention by all means is first and foremost benefit the patient. Artificial joint infection diverse tracking volunteers at the law made mandatory registers will bring problems of infections and the number of registers in the quality of the evaluation.The purpose of this study was to determine whether data documented and stored in a patient record system can be used to predict or explain possible risk factors for artificial joint infections. The target group of this study consisted of patients who had undergone an artificial joint infection and a control group included arthroplasty patients who had not suffered an infection. The patients were treated in University Hospital in the years 2011-2013. According to the results, the patients who underwent a postoperative infection were prone to still experience surgical wound leakage at the time of their discharge from the hospital. In addition, special attention should be paid to patients’ perioperative and post-operative thermoregulation. Artificial joint infections are somewhat predictable and preventable though taking risk factors into account, but further studies on the topic are still needed. Variable documentation still requires of the development and standardizes.
